New Patients

Joining the practice is easy. All eligible people living in the practice’s catchment area can register with South View Partnership as a regular patient. You can check the catchment area using the link below.

If you do not have an address of your own, please state upon the new patient form that you do not have an address. You do not need to provide proof of your address, photographic ID or immigration status to register with us. After you have completed and submitted your registration forms your registration will be processed.

We may ask for confirmation of identity, addresses etc to help us trace your previous medical records, but you do not need to provide these unless we ask for them.
